General Policy Consent - 1. Aloha Pet Services, LLC., will endeavor to create as safe an environment as possible for the training and boarding care of the dog(s) identified above (the “Dog”), and will offer only sound, safe, and responsible training, care, and post-boarding training instructions. Owner agrees, however, that all animals may exhibit unpredictable behaviors and that Aloha Pet Services will not be liable for the actions of the Dog that are outside of the control of Aloha Pet Services. Owner agrees to allow random stool samples to ensure safety of the kennel. Owner agrees that the recommendation by Aloha Pet Services of any product or service is not a guarantee of performance or owner’s satisfaction with that product or service. 2. Aloha Pet Services, LLC., agrees to indemnify and hold owner harmless from and against any claims of injury, expense, costs, loss, or damages caused by the gross negligence or willful misconduct of Aloha Pet Services while the Dog is in the sole custody, care, and control of Aloha Pet Services. Owner acknowledges that Aloha Pet Services has no control over the Dog when the Dog is not in the exclusive custody, care, and control of Aloha pet Services, and owner agrees to indemnify and hold Aloha Pet Services harmless from and against any and all claims of injury, expense, costs, loss or damages caused by the actions of the Dog when the Dog is not in the sole custody, care and control of Aloha Pet Services. 3. Owner acknowledges and agrees that owning any animal, including the Dog, has inherent risks. These risks include but are not limited to the risk of dog bites to owner. 4. Owner fully understands the tools and techniques that are used to train and modify the behavior of their dog. Owner agrees to the tools and techniques which include Prong Collar, Bark Collar, Remote Training Collar, Bonker, Slip Lead, Muzzles, Flat Buckle collar, and/or Martingale collar. *Yes, I agree to the above general policyNo, I do not agree to the above general policy

Payment and Cancelation Policy - Payment for all sessions will be submitted online. We accept all major credit cards. If you cancel your session, you will receive a credit back so you can reschedule your training. However, no money will be returned to you once your sessions are paid for. Once you submit your payment, we will reach out to you via e-mail to schedule a time and day for your dog training sessions. It is important that you be on time to your session(s). NOTE: If you are late to the appointment, the time missed will not be made up. Photo/Video Release Policy Consent (All Clients) - I hereby assign and grant Aloha Pet Services, LLC., the right and permission to use, reproduce, distribute, and publish the photograph(s), film(s), videotape(s), audio and video recording(s), electronic representation(s) and/or sound recording(s) made of me and/or my dog(s) at any time during the training and/or boarding of my dog(s) by Aloha Pet Services. I hereby release Aloha Pet Services from any and all liability resulting from such use, reproduction, distribution, and publication. I hereby authorize the reproduction, sale, distribution, copyright, exhibit, broadcast, and/or electronic storage of any and all such photograph(s), film(s), videotape(s), audio and video recording(s), electronic representation(s) and/or sound recording(s) without limitation at the discretion of Aloha Pet Services and I specifically waive any right to any compensation I may have for any of the foregoing. *Yes, I agree to the above photo/video release policyNo, I do not agree to the above photo/video release policy
